{"id":7662,"date":"2023-11-05T19:03:30","date_gmt":"2023-11-05T18:03:30","guid":{"rendered":"https:\/\/dermatim.rs\/?post_type=dermatologija&#038;p=7662"},"modified":"2023-11-14T14:57:00","modified_gmt":"2023-11-14T13:57:00","slug":"herpes-zoster-shingles","status":"publish","type":"dermatologija","link":"https:\/\/dermatim.rs\/en\/dermatology-in-belgrade\/herpes-zoster-shingles\/","title":{"rendered":"Herpes Zoster (shingles)"},"content":{"rendered":"\r\n\r\n<strong>Herpes zoster<\/strong> (<strong>HZ<\/strong>), also known as shingles, is caused by the reactivation of the <strong>varicella-zoster virus<\/strong>.\r\nVaricella-zoster virus is transmitted by droplet spread. It is a primary infection, usually developing in children younger than 15, commonly known as <strong>chickenpox<\/strong>.\r\n\r\n \r\n\r\nThe disease is usually mild and initial symptoms like fever, malaise, conjunctivitis quickly pass and then small blisters appear on the trunk, head, proximal and distal parts of the arms and legs. Lesions are accompanied by a strong itching sensation. Over time, they turn into scabs, which fall off after a few days, leaving slight recessed scars. The pain can be so strong that it resembles the attack of renal colic. Sometimes there are also sensations of itching, numbness, increased sensitivity of the skin or tingling. After a couple of days, redness with or without swelling will appear in the same area with clustered smaller or larger blisters.\r\nThe contents of the blisters will become darker over time, and then hemorrhagic crusts are formed. After they fall off, the scars will remain present.\r\n\r\n \r\n\r\nThe most common complication of Herpes zoster is <strong>postherpetic neuralgia<\/strong>. It is an inflammation of the nerve fibers, manifested by severe pain or burning, tingling or numbness of the affected part of the skin. The problems can endure up to six months after skin lesions resolve and are quite resistant to therapy.\r\n\r\n \r\n\r\nIf the ophthalmic nerve is affected by infection, the so-called <strong>herpes zoster<\/strong> <strong>ophthalmicus<\/strong> develops, which can lead to keratitis and eye complications. When the auditory nerve is affected, it can cause hearing loss, ear pain with or without dizziness.\r\nFacial nerve paralysis is rarely observed.\r\n\r\n \r\n<h2 id=\"headline-12-460\" class=\"wp-block-heading\">Herpes zoster \u2014 How it is diagnosed?<\/h2>\r\n \r\n\r\nThe diagnosis is usually made based on anamnestic data and unilateral localisation of typical skin lesions.\r\nWhen it comes to laboratory testing, <strong>Tzanck smear<\/strong>, <strong>PCR<\/strong> or <strong>DFA<\/strong> (Direct fluorescent antibody) can be used.\r\n\r\n <div class=\"wp-block-image\">\r\n<figure class=\"aligncenter size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"685\" class=\"wp-image-794\" title=\"Herpes simplex on the arm\" src=\"https:\/\/dermatim.rs\/wp-content\/uploads\/2020\/11\/dermatim-herpes-simplex-2-1024x685.jpg\" alt=\"Herpes simplex on the arm\" srcset=\"https:\/\/dermatim.rs\/wp-content\/uploads\/2020\/11\/dermatim-herpes-simplex-2-1024x685.jpg 1024w, https:\/\/dermatim.rs\/wp-content\/uploads\/2020\/11\/dermatim-herpes-simplex-2-300x201.jpg 300w, https:\/\/dermatim.rs\/wp-content\/uploads\/2020\/11\/dermatim-herpes-simplex-2-768x514.jpg 768w, https:\/\/dermatim.rs\/wp-content\/uploads\/2020\/11\/dermatim-herpes-simplex-2.jpg 1494w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><figcaption class=\"wp-element-caption\">Herpes simplex on the arm<\/figcaption><\/figure>\r\n<\/div> \r\n<h2 id=\"headline-14-460\" class=\"wp-block-heading\">How is Herpes zoster treated?<\/h2>\r\n \r\n\r\n<a href=\"https:\/\/dermatim.rs\/en\/dermatology-in-belgrade\/herpes-simplex-skin\/\">Treatment of Herpes<\/a> zoster involves the use of local and systemic drugs. Apart from dermatologists, a treatment is often prescribed also by ophthalmologists and neurologists. It is optimal if treatment is started within 72h of the appearance of skin lesions.\r\nIn systemic therapy, <strong>Acyclovir<\/strong>800 mg \/daily for 7 to 10 days is prescribed; <strong>Famciclovir<\/strong> 500 mg 3 times daily for 7 days or <strong>Valaciclovir<\/strong>1 g 3 times daily for 7 days.\r\n\r\n \r\n\r\nWhen it comes to topical preparations, it is recommended to use antiseptics, desiccant agents, as well as antibiotic ointments in case of development of secondary bacterial infection.\r\n\r\n \r\n\r\nPostherpetic neuralgia can be treated with low doses of tricyclic antidepressants; antiepileptics \u2014 Gabapentin, Pregabalin; short-term administration of systemic corticosteroids; use of 8% Capsaicin patch and\/or local anaesthetics.\r\n\r\n","protected":false},"excerpt":{"rendered":"<p>Herpes zoster (HZ), also known as shingles, is caused by the reactivation of the varicella-zoster<\/p>\n","protected":false},"author":14,"featured_media":0,"comment_status":"closed","ping_status":"closed","template":"","class_list":["post-7662","dermatologija","type-dermatologija","status-publish"],"acf":[],"_links":{"self":[{"href":"https:\/\/dermatim.rs\/en\/wp-json\/wp\/v2\/dermatologija\/7662","targetHints":{"allow":["GET"]}}],"collection":[{"href":"https:\/\/dermatim.rs\/en\/wp-json\/wp\/v2\/dermatologija"}],"about":[{"href":"https:\/\/dermatim.rs\/en\/wp-json\/wp\/v2\/types\/dermatologija"}],"author":[{"embeddable":true,"href":"https:\/\/dermatim.rs\/en\/wp-json\/wp\/v2\/users\/14"}],"replies":[{"embeddable":true,"href":"https:\/\/dermatim.rs\/en\/wp-json\/wp\/v2\/comments?post=7662"}],"wp:attachment":[{"href":"https:\/\/dermatim.rs\/en\/wp-json\/wp\/v2\/media?parent=7662"}],"curies":[{"name":"wp","href":"https:\/\/api.w.org\/{rel}","templated":true}]}}
